A Support at Home service agreement is the written agreement between you and your chosen provider. It explains what support you will receive, how much services will cost, and what you and the provider are each responsible for.
What should the agreement include?
Your agreement should clearly list the services you have agreed to receive, the prices for those services, how participant contributions will be handled, and how changes will be discussed with you. You should receive a copy of the signed agreement for your records.
Take time to understand it
You can ask questions, request information in a format you understand, and involve a family member, registered supporter, advocate or other trusted person. Do not feel rushed. Make sure the services and prices match your needs, goals and available budget.
Agreements can be reviewed
For ongoing services, the agreement must be reviewed at least every 12 months. You can also ask for a review if your needs change, if you want different services, or if you do not understand a proposed change. Your provider should explain changes clearly and work with you to reach agreement.
Choosing or changing a provider
You can compare registered providers and choose one that can deliver the approved services you need. You can also change your Support at Home provider. Before changing, check your current agreement, agree on an end date, choose a new provider, and arrange a new service agreement so your care continues smoothly.
